Unresectable Recurrent Multiple Meningioma: A Case Report with Radiological Response to Somatostatin Analogues

چکیده
Medical treatment of meningiomas is reserved for cases in which surgery and radiotherapy have failed. Given that a high percentage of meningiomas express somatostatin receptors, treatment with somatostatin analogues has been proposed. In addition, these medications have been shown to have an antiproliferative and antiangiogenic effect in vitro. To date, very few cases with clinical response and none with radiological response have been described. The case described here is the first to report a radiological response. A 76-year-old Caucasian male was first diagnosed with unresectable meningioma at age 47. The patient experienced multiple recurrences and underwent three surgeries and radiotherapy over the years from the initial diagnosis. Despite treatment, the disease continued its progression. Based on an Octreoscan positive for tumour uptake, therapy with extended-release somatostatin analogues was started. Although no clinical neurological improvement was observed, magnetic resonance imaging scans revealed a discreet but continuous radiological response over time. After >2 years of continuous administration of lanreotide, the patient remains progression free. In highly selected cases, somatostatin analogue treatment for meningioma may be benD ow nl oa de d by : 54 .1 91 .4 0. 80 9 /1 7/ 20 17 9 :5 1: 30 A M Case Rep Oncol 2016;9:520–525 DOI: 10.1159/000448212 © 2016 The Author(s). متن کاملMultiple High Grade Rhabdoid Papillary Meningiomas Mimicking Choroid Plexus Carcinoma: A Case Report
Rhabdoid papillary meningioma is an uncommon aggressive variant of meningioma which has the potential to metastasize and spread throughout the brain and even out of the cranium. Herein, we present recurrence of the brain tumor in a 26-year-old woman. The patient had history of the surgery for two lesions in the right temporal lobe and the left cerebellopontine angle. Imaging showed three lesion...
متن کاملPET/CT detection of multiple extracranial localizations in a patient with anaplastic meningioma
We report herein a case of a 65-year-old male with intracranial recurrence of atypical meningioma initially treated with a combination of surgical resection and gamma knife radiotherapy. Afterwards, he underwent a Ga-DOTANOC PET/CT scan in order to evaluate the feasibility of peptide receptor radionuclide therapy (PRRT). The scan identified multiple pulmonary, pleural and lymph node localizatio...

متن کامل68Ga-DOTA-D Phe1-Tyr3-Octreotide (DOTATOC)-PET/CT in a Suspected Case of Recurrent Meningioma
We present a case of 45-year-old female who presented with headache for last 1 month. She was a follow-up case of left frontal meningioma that was operated 4 years back. CEMRI of the brain suggested post-surgical changes with no obvious enhancing residual/recurrent lesion. The patient was then referred for 68 Ga DOTANOC PET/CT to rule out recurrent disease. PET/CT revealed a focus of somatostat...
